Insurance
Insurance companies offer a variety of different plans, each with different benefits, and there are times when the recommended treatment may not be covered, even if we participate in the network. For this reason, it is always best that you contact your insurance company for concerns regarding exact benefits and co-insurance. Keep in mind, your insurance is a contract between you, your employer, and your insurance company.
Any estimates for payment are simply based on coverage, such as deductibles, co-pays, maximum benefits and/or non-covered services. Payment due is expected at the time services are rendered.
We can only provide an estimate of how much your insurance will cover towards treatment. Every insurance plan has limitations, exclusions, and complexities that may render estimates of coverage inaccurate.
Please keep in mind that dental insurance does not usually cover your total bill. Any insurance estimates are only estimates, and you are responsible for all fees in their entirety.
At your request, we may submit a pre-determination of benefits to insurance. Processing may take several weeks, which may delay treatment, but we are happy to provide this service to all of our established patients.
We are in-network providers with several of the major PPO insurance plans. However, the world of dental insurance is complex, with multiple PPO sub-plans offered by the same insurance provider. Please always check with your insurance company to see if we are in network. Feel free to call our office at 847-676-3636 with your insurance information and we will do our best to provide you with an estimate.
